Initially, he was approved for Long Term Disability benefits. Then, after 10 months, Mutual of Omaha abruptly terminated his benefits.

A customer service representative from Arkansas began to lose feeling in his feet. Eventually, as a result of complications with diabetes, he had three toes amputated.
