A sourcing analyst in Indiana applied for Long Term Disability benefits after stopping work due to serious health conditions, including a stroke, congestive heart failure, hypertension, diabetes, and vocal paralysis. Symetra Insurance Company denied the claim, asserting that full-time work in the regular occupation remained possible.

With income at stake and appeal deadlines approaching, the claimant turned to Fields Law for legal guidance and a clear path forward. The Fields attorneys reviewed the insurer’s claim file and strengthened the record by obtaining targeted medical records and physician reports. After the appeal submission, Symetra Insurance Company overturned the denial and approved the claim, resulting in a lump-sum payment for past-due benefits and ongoing Long Term Disability benefits.

Donald – Occupational Therapist – Columbus, Indiana

Chronic Inflammatory Demyelinating Polyneuropathy

An occupational therapist in Indiana developed significant symptoms, including low back pain, chronic radiculopathy, chronic insomnia, chronic migraines, and chronic inflammatory demyelinating polyneuropathy. A Long Term Disability claim through Cigna was approved, and benefits were paid for more than two years. After Cigna required an examination with a physician of its choosing, the insurer asserted the disabling condition was mental rather than physical and terminated benefits, stating that the plan’s mental health limitation had been exhausted.

Fields Law attorneys prepared a detailed administrative appeal that addressed the insurer’s rationale directly and cited the medical record to confirm a physical basis for the disability. The appeal also included updated medical documentation describing daily symptoms and functional limitations. After review, Cigna reversed the termination, paid past-due benefits, and resumed ongoing monthly payments.
