We've got you covered

Scott – Machinist – Richmond, Indiana

Asthma, Long Covid

A machinist in Indiana developed disabling asthma and long COVID symptoms that made continuing work impossible. Although Long Term Disability benefits were initially approved, New York Life later terminated the claim, cutting off the income support the policy was intended to provide.

Fields Law prepared a focused appeal, gathering medical records documenting the severity and ongoing impact of the conditions, including a detailed report from a treating physician. A Fields attorney drafted a legal memorandum explaining why reinstatement was required under the policy terms. The appeal resulted in the claim being reapproved, with back pay issued for benefits owed since the termination, and Long Term Disability benefits restored. Fields professionals continue to monitor the claim to help maintain the benefits in place.

Marisa – Territory Manager – Elkhart, Indiana

Chemo Induced Peripheral Neuropathy, Pain Med Induced Grogginess, Chronic Pain, Chronic Weakness, Immobility, Assistive Ambulatory Device, Cane

A territory manager for a major pharmaceutical company from Indiana developed several complicated conditions, limiting her mobility and use of her upper extremities. Unable to continue working, she applied for Long Term Disability benefits. She was approved, and her benefits ran for a year before New York Life terminated her claim.

Knowing she needed help, she retained Fields Law. The Fields attorneys began assembling her appeal, including gathering her updated medical records and securing letters of support from her primary treating providers. Upon reviewing the appeal, New York Life overturned their prior denial and reinstated benefits.

Charles – Software Engineer – Leesburg, Virginia

Vestibular Migraines, Dizziness, Vertigo, Herniated Disc

A Software Engineer began to experience worsening migraines. He had dealt with migraines before, but these were bad enough to make work impossible. He could no longer drive and had intense bouts of vertigo. His condition forced him to stop working. Subsequent tests revealed several herniated discs in his back. His doctors were in regular contact with each other to coordinate his treatment, and he applied for Long Term Disability benefits, based on his medical records. His insurer denied the claim despite the medical evidence.

Frustrated and worried about supporting his family without income, he contacted Fields Law Firm to assist with an appeal. Our attorneys collected records from five physicians who had treated our client and created a timeline of treatment that New York Life could not ignore. We obtained new vocational reports showing that our client could not go back to work due to his symptoms. With the new reports, our attorneys made a strong case that New York Life’s initial decision was wrong. Our client won his appeal and was awarded back-pay and ongoing benefits. He was relieved to have the process complete and was happy with the result.
