Eric – Main Router – Gary, Indiana

Lower Extremity Dysfunction, Chronic Back Pain, Lower Extremity Weakness, Degenerative Disc Disease, Sciatic Nerve Damage

A Main Router at an Amazon warehouse in Indiana developed serious dysfunction in his low back and legs from working on his feet all day over the course of years. Unable to continue working his light level occupation, he left work. He applied for Long Term Disability benefits and was approved by the insurer. He remained on claim for two years until the change in his policy’s Definition of Disability, when the insurer terminated his benefits claiming he could return to work in an alternate occupation.

After waiting several months, he retained Fields Law to fight the insurer. On a tight deadline, the Fields Law team submitted an appeal of the denial, but the insurer upheld their decision on reconsideration. Our attorneys then filed a lawsuit against the insurer in federal court. Faced with mounting litigation, the insurer made a settlement offer which the Fields Law attorneys managed to negotiate into a favorable lump sum settlement for our client.

Stephen – Quality Control Supervisor – Milan, Indiana

Anxiety, Depression, Bipolar II

A quality control manager became disabled due to severe anxiety, depression, and bipolar symptoms. After several months of mental health treatment, his doctors recommended he leave work to focus on treatment. His Long Term Disability insurance company fought his claim from the very beginning, claiming that he was able to perform his job.

He hired Fields Law to sue the insurance company. The insurance company initially tried to settle the case against our client for a very small amount of money. After several rounds of negotiation, the insurer agreed to pay our client a large portion of the benefits he was owed throughout the life of his claim. Our client was very happy with the representation he received and glad he chose Fields Law.

Ty – Truck Driver – Merrillville, Indiana

Radiating Back Sain, Spondylosis, Chronic Neck Pain, Chronic Regional Pain Syndrome (CRPS)

A truck driver from Indiana needed surgery after an ankle injury. The surgery exacerbated long-standing issues with back and neck pain. He could no longer stand or sit for more than a few minutes at a time. He needed help with basic hygiene, putting on his shoes, and preparing food. It was impossible for him to go back to work. Once his Short Term Disability expired, he applied for Long Term Disability benefits. His employer-provided insurance package approved him for monthly benefits however, after receiving benefits for eleven months, Liberty Mutual stopped paying because he no longer fit their definition of disability.

Worried about his lost income, and still needing help for everyday tasks, he called Fields Law for help. Our support team quickly gathered all of our client’s medical records and the claim file from Liberty Mutual. The Fields Law attorneys reviewed all the evidence and ordered supplemental reports from a vocational specialist who showed that there was no way our client could return to work. Our attorneys sent an appeal to Liberty Mutual with the strengthened evidence and applicable legal arguments. The insurance company changed their decision, and our client received a lump-sum for back pay and ongoing monthly benefits.
