A director of software in Louisiana stopped working when Crohn’s disease, nonalcoholic steatohepatitis (NASH), and neuropathy made it impossible to perform job duties on a consistent basis. A Long Term Disability claim was submitted under an employer-sponsored policy with Mutual of Omaha, but the insurer denied the claim, asserting that the medical conditions were not severe enough to qualify for benefits.
Fields Law moved quickly to build a clear, document-driven appeal. Treating provider records were obtained, supportive physicians were asked to prepare detailed narrative reports addressing functional limits and work capacity, and an attorney prepared a comprehensive appeal memorandum explaining why the policy terms and medical evidence supported approval. After the appeal was submitted, Mutual of Omaha reversed the denial, monthly benefit payments began, and back pay was issued dating to the initial denial.

A pharmacy manager was diagnosed with bilateral breast cancer. She stopped working to undergo a double mastectomy and aggressive chemotherapy treatment.
